Neighborhood Connections to Health

Neighborhood Connections to HealthNeighborhood Connections to Health (NCTH) is a nonprofit organization based in the Greater Freehold Area, working to address the social determinants of health in a preventative fashion. We began our work as a coalition in 2016 in Freehold Borough, NJ to address the major health disparities experienced by residents here. Through focus groups with residents and evidence-based research, we developed our 4 committees: Access to Healthy Food, Lead Poisoning Prevention, Intergenerational Activities, and Bikeability Walkability. In addition, our Blueprint for Action was created and continues to guide NCTH in improving the health of Freehold Area residents by addressing underlying health factors that influence health outcomes. We work alongside residents, organizations, municipalities, and stakeholders to build a Culture of Health in our community. NCTH is participatory; not only does the community benefit from change but they are welcome to be part of it. Our vision is to expand into Monmouth County and continue being the connection to healthy living for all constituents.
