Procedures for the handling of motor vehicle crashes not investigated at the scene:
- The individual reporting the alleged crash shall use the online SR-1 application (Self Reporting Motor Vehicle Crash Report) found at the following link: https://njdot.prod.simpligov.com/prod/Portal/ShowWorkFlow/AnonymousEmbed/a24cb054-6900-4ab2-97c8-67f1f64ae7a0
- If this a change report for an existing SR-1 application please use the following link to make changes: https://njdot.prod.simpligov.com/prod/Portal/ShowWorkFlow/AnonymousEmbed/90266f0b-c759-4595-8572-df78fc45b739
- The site can also be found by entering NJSR1 into any internet search engine.
- If a driver is involved in a Hit and Run accident that is not investigated by the police and they wish to pursue the driver and have the license plate, they can obtain owner and insurance information from the New Jersey Motor Vehicle Commission. The request form can be found online at www.state.nj.us/mvc. Scroll to the bottom of the page and click Forms, under the heading Request Records select DO-11A (Vehicle Registration Application Request).
